This field displays the date and time the injury was reported.

The employee ID number of the person who the injury was reported to.

This field displays the employee type, i.e. Internal/External.

This field displays the employee's ID number.

The person who witness the injury can be recorded in this field.

Enter the number of hours worked on the day prior to the incident occurring.

Select from the dropdown the degree of Incapacity.

Enter the percentage of the incapacity of the injury.

The attending medico can be recorded in this field.

• This field can hold up to 25 characters.

Enter the telephone number of the attending medical person.

Select the type of treatment required at the time of the injury. The standard list consists of the following:

• First Aid
• Hospitalisation
• Medical Officer
• None

Select from the dropdown the status of the claim in relation to the injury. The standard list consists of the following:

• Received from Employee
• Lodged with insurer
• Approved
• No Approved (Closed)
• No claim to be lodged
• Unknown
• Finalised
• Legal Action

This field displays the date the claim status is changed

Enter a claim number for the injury.

Enter the date the first symptom occurred for the injury.

Is this injury legally reportable?

Is this injury a fatality?

Does this injury require the person to attend a rehabilitation program?
